If you want to be considered for a grant, send us an application containing the items specified below. You will make your best case by giving us the information we need at the outset; we will then contact you if we have questions or additional concerns. Please submit all your written materials before calling to discuss your ideas.
We will acknowledge your application within two weeks of its receipt. If you do not hear from us within three months, or if you have questions, please e-mail Judith Lopez.
Your application should contain:
A cover letter, on your stationery, signed by your director
A completed New York Community Trust Proposal Cover Sheet (*please see bottom of this page to download this document)
A proposal (no more than ten pages) that includes:
Agency Background (mission, major activities, and credentials for carrying out project)
Project Description
Brief statement of problem to be addressed
Goals and objectives
Target population
Project activities
Staff (append their rèsumès)
Plan for measuring project results
Project Budget (expenses and income)
Agency Operating Budget
A list of the Board of Directors
Latest Audited Financial Statement (if available)
Current Annual Report (if available)
Affirmative Action Policy
